NOMINATE A MOSQUE

Welcome to the Masjid1001 Nominate A Mosque form. Before you begin filling out the form, please ensure that you have the following requirements ready to be uploaded:

1. Letter from Mosque Management: Please upload a letter from the management of the mosque stating that they approve of the repair nomination and that they are aware of the repairs needed. This letter should be on the official letterhead of the mosque, signed by the authorized personnel, and dated.
2. List of Cost Description/Breakdown: Please provide a detailed list of the cost breakdown for the repairs needed. This should include a description of each repair, the estimated cost, and any additional information that may be relevant.
3. Images of the nominated Mosque: Please upload clear and high-quality images of the mosque that requires repair. These images should clearly show the areas of the mosque that need repair and the extent of the damage.

Once you have all the necessary requirements ready, you can proceed to fill out the online form.
